Criteria for nominating a family:

  • Community Residency: The nominated family must live within Hampton Roads (757) or Puerto Rico (787).

  • Relationship Requirement: Nominators should know the extent of the family’s situation regarding cancer or disability. (Families can nominate themselves.)

  • Challenges Faced by the Family: Families facing hardships and challenges related to disabilities, caretaking responsibilities, or battling cancer.

  • Detailed Nomination Information: Nominators should provide comprehensive details on the nomination form, so SEEN can assess the family as a candidate for the program.

  • Supporting Documentation: Nominated families may need to submit documentation to support the difficulties they are facing.

  • Support from Nominator: We encourage nominators to actively participate within our SEEN team by helping share the family’s needs throughout our community. For example, you could share social media content and enhance awareness.
